Alpha Sigma Zeta Scholarship Application:
Please find below, a scholarship cover letter and application for seniors. One award will be presented in the amount of $1,000. Below is a summary of the information included in the cover letter and is provided here for convenience.
· At least a 3.0 GPA on a 4.0 scale
· At least 50 clock hours of service performed January 2019 through December 2019
· Official transcript verifying GPA and ACT Score mailed to 1013 8th Ave. W, Birmingham, AL 35204
· Signed documentation verifying the dates, times, and location of services rendered
· Completed application and documents submitted or postmarked by Tuesday, January 21,2020 to alphasigmazetascholar@gmail.
The completed application and verification service hours may be submitted via mail as an attachment or via ground mail. Applicants who wish to use the fillable version of the form must have the latest version of Adobe Acrobat Reader DC in order to fill out the application. They can download it for free at: https://get.adobe.com/reader/ . Transcripts must be mailed. Documents containing Personal Identifiable Information will be shredded once the monetary award has been mailed.
